If you are medically qualified to reside in a nursing home, have financial need, and are 21 years or older — but prefer to live at home — you may have a wide variety of services available to you.

What is Community Health Choice?

In Pennsylvania, community health choice  programs or Home and Community Based Services (HCBS),  provide support services to qualified individuals in need meeting special financial and medical criteria.

Community Health Choice  programs can be hard to understand. Each program has its own special rules of who is served, and how to apply, the criteria that must be met in order to qualify, and the services provided.

At no cost to you, we’re here to counsel you through the approval process and, after you are approved, coordinate your benefits.

  • Meal Delivery (Hot or Prepared)
  • Personal Emergency Response (PERS) System
  • Respite Care Services (Temporary Caregiver Relief)
